This memo outlines our insurance and payment policies to ensure clarity regarding your dental care expenses.

We are a participating provider with Ameritas, Excellus BCBS PPO, and UMR.

​

Please understand that even with participating plans, not all treatment is paid for in full, and you will incur some out-of-pocket expenses. Your insurance company and employer determine your specific coverage. We are happy to provide an estimate of your cost in advance of your visit upon request. This is an estimate, and any remaining balance after your insurance processes the claim will be sent to you in a statement, due within 30 days. Your estimated portion is due at the time of treatment.

For posterior (back teeth) resin (white) fillings, some insurances do not cover the full cost. In these cases, we ask you to pay your estimated portion at the time of service. For specific details on how your insurance covers posterior resins, please contact your insurance provider directly. We can also submit a predetermination of benefits on your behalf.

If your dental plan is with an insurance company we do not participate with, we will still submit the claim for you. If we have not yet established a relationship with your particular insurance, we may request payment in full at the time of service. If a relationship has been established, we will ask for your estimated out-of-pocket portion at the time of your visit.
